Where is the upload form to upload my own iPhone title to the app store?

In the US, it's http://itunesconnect.apple.com. The simplest way to get there is to log into Apple's developer portal http://developer.apple.com and click on the "iTunes Connect" link in the sidebar.

You can simply upload apps to iphone from itunes.

You do it from iTunes but beware, it's not free. To become a developer it's like 100$ registration fee, then 100$ a year. I'm not exactly sure if this is correct but I think it is.
