Android "privacy policy"

I am about to publish a game to Google Play. My game doesn't use advertising and it's free. I am trying to provide "Privacy policy" document. Does unity collect any information by default? Is there (is anyone willing to share) any template document for this. Like: This game is free and doesn't collect any information.Some information might be automatically collected by Google Play and other services. Something like this...
