FPSPlayer script from FPS tutorial won't work...

The FPSPlayer script from the FPS tutorial has suddenly stopped working. Usually when I die, the screen will fade white and the scene will reset, but now when the player dies the screen instantly changes to white (no fading) and stays that way without reloading the scene. Can someone please help me and tell me what to fix this error?
