How to Make a Godlooking SlenderGame

How can i make a Slender Game that when i take a Page Slender Teleports closer and also how can i make this Screen Effect where when you look at Slender this Creepy effect comes out?

P.S. i want Slender to Teleport not just move over Ground with cause then it will be pretty easy i would be coller if it Teleports somethimes infront of my face!
