Rotation and position centres of blender model are wrong.

You can either put this model into an empty game object as a child, or you can edit its rotation in Blender.
Blender also allows you to rotate coordinate axes during export, if you need to translate between X, Y, and Z at right angles (like XYZ -> YXZ).
